Schedule C is an add-on form to Form 1040 to report income and expenses from self-employment. Schedule C is filled out for different reasons, these are:

1. Self-employment income.

2. Income from a business registered as a sole proprietor (DBA).

3. Income from a business registered as an LLC with only 1 member.

4. Commission income.

5. Uber or Lyft.

6. Other.

Schedule C is required to be filled out if you have generated any of the income mentioned above.

This service includes:

1. Preparation of Schedule C.

This service does not include:

1. Preparation of Form 1040 or the tax return.

2. Any service not mentioned in the included services is not included.

3. Bookkeeping preparation is not included if necessary. If Bookkeeping service is necessary, the cost will be quoted and billed separately.

Important:

- If you are going to purchase the Annex D service, you should know that you must also have added the service of preparing the Form 1040 tax return (taxes) since Annex E cannot be done separately from Form 1040.
